There were 991 complaints about Heart of England NHS Foundation Trust, which also runs Good Hope and Heartlands Hospital.
Only North Staffordshire Hospital NHS Trust had more, 1,824.
Heart of England is one of the largest trusts in England, serving more than a million people.
University Hospitals Coventry And Warwickshire NHS Trust serves a similar number and saw 484 complaints last year, 2009/10.
National figures are the highest since records began 12 years ago.
NHS Information Centre chief executive Tim Straughan said: “It is important to bear in mind that there has been a substantial increase in NHS activity in England over time.”
