A PROBE has been launched into a patient death after a tube which is inserted via the nose was ‘misplaced’.

Health chiefs have classed the Heartland incident as a ‘never event’ – those which are ‘serious’ and ‘largely preventable’.
A Solihull NHS Care Trust report said the incident involved a ‘misplaced nasogastric tube’.
The tube is placed through the nose, past the throat and into the stomach.
The report says: “The client is a Solihull resident and has since died.”
The NHS Birmingham East and North authority is investigating and the care trust said it will be kept informed.
Official NHS guidance states: “Never Events are serious, largely preventable patient safety incidents that should not occur if the available preventative measures have been implemented.”
