HEALTH chiefs have launched a probe after staff failed to follow up a patient’s lung x-ray more than a year before their death.

Heart of England NHS Foundation Trust, which runs Solihull Hospital, has launched a serious untoward incident investigation over the patient’s care.
An NHS report says the patient underwent a chest x-ray in September 2008 which found a lung lesion, a tissue which can be cancerous.
This was reported to the radiology department ‘but no further action taken’ the report says.
The patient re-attended Good Hope Hospital in Sutton Coldfield, part of the trust, in January complaining of abdominal pain and later died.
No further information was published. The investigation has yet to be completed.
